Foundational
Assessment
The Foundational Assessment is where every patient begins. It gives us the time to really understand what’s going on through comprehensive lab testing and a 75–90 minute visit with Susan. We’ll talk through how you’re feeling, what your results are telling us, and what you want to work toward. You’ll leave with a clear understanding of your results, your options, and a plan for what comes next.
The Foundational Assessment stands on its own, with no obligation to continue into Ongoing Care.

Ongoing Care
Ongoing Care is available after you complete your Foundational Assessment if you want continued support putting your plan into practice. Over 12 months, we’ll follow your progress, review labs, adjust your plan when needed, and support you along the way with Susan and your health coach. Ongoing Care is a separate service.
The Foundational Assessment fee does not apply toward the membership.
